Las mutaciones en el gen PINK1 en pacientes con Parkinson juvenil se asocian al desarrollo de cuerpos de Lewy, según ha constatado un equipo multicéntrico en tres generaciones de una familia canaria con más de 20 miembros afectados.
El grupo de José Matías Arbelo, jefe de la Unidad de Parkinson y Trastornos del Movimiento del Servicio de Neurología del Hospital Universitario Insular de Gran Canaria, ha estudiado a miembros de tres generaciones de una misma familia con Parkinson juvenil.
En este trabajo también han participado Pau Pastor, del Servicio de Neurogenética de la Clínica Universidad de Navarra, e Isidro Ferrer, del Servicio de Anatomía Patológica del Hospital de Bellvitge, de Barcelona. Se trata de una mutación autosómica recesiva, por lo que hay sujetos portadores de la mutación PINK1 que aún no han desarrollado el Parkinson juvenil. Es una forma de la neurodegeneración que debuta antes de los 30 años. De hecho, al paciente más joven de la familia se le diagnosticó el Parkinson con 14 años.
Lenta evolución
Una de las ventajas que tiene esta forma es su lenta evolución y la buena respuesta que tiene al tratamiento con L-dopa. “Es diferente decirle a un chico de 14 años que tiene Parkinson que va a llevar una vida casi normal, que se puede controlar bien su enfermedad y que la evolución es lenta, en lugar de que su patología le incapacita para tener una buena calidad de vida”.
José Matías ha comentado que el descubrimiento de las mutaciones asociadas a Parkinson ayuda a tener un mayor y mejor conocimiento de la fisiopatología de la enfermedad. Con ésta en el gen PINK1, ya hay descritas trece mutaciones asociadas al desarrollo de Parkinson juvenil.
Después de analizar el ADN en los 20 miembros de esa familia afectados, se observó que presentaban una mutación en el gen PINK1, “que ya estaba descrito en la literatura, pero nosotros hemos visto que dicha mutación produce una alteración que se asocia a la formación de cuerpos de Lewy, y eso no estaba descrito”, ha apuntado Matías Arbelo a Diario Médico.
